A valid prescription is required for the Serene 5 Oxygen Concentrator to comply with regulatory requirements before shipment.


If you have purchased or plan to purchase the Serene 5, please upload your prescription using the form below or send it to our support team. We will process and ship your order once your prescription is verified.

 

We accept photos, scans, PDF, JPG, and PNG files.
Your prescription must be issued by a licensed healthcare provider and include:

  • Patient name
  • Prescribed oxygen flow rate (if applicable)
  • Healthcare provider information
  • Provider signature or authorization
  • If you have already placed an order, please include your order number to speed up processing.
